The Local 888 Health and Welfare Fund is a Taft-Hartley plan. This means that one or more employers contribute to the plan; the plan is collectively bargained with each participating employer; the plan and its assets are managed by a joint board of trustees equally representative of labor and management, and all assets are placed in a trust fund. The Union negotiates for monthly contributions towards health and pension benefits. Contribution amounts may differ among employers. Plan rules determine whether the benefits provided to members will differ based on each employer’s contribution.

The Local 888 Health and Welfare Fund is self-funded, which means that we can provide a variety of plans to suit our members needs. If you have any questions about your medical or pension plan, please contact the Health Fund.

Dear Members:

Please be advised that Open Enrollment is during the month of February. As you know, Open Enrollment is the time in which you are given the opportunity to add and/or delete your eligible dependent (s) without a qualifying event. Eligible dependents are spouse, children under the age of 19, and children 19-23 who are full time college students. In order to add your eligible dependents we need a copy of all pertinent documents (eg: marriage certificate, birth certificate (s), and full-time college letter for children 19 or older). In order to delete a dependent, you must send a written request. If you fail to provide the Local 888 Health Fund with the required document (s) before March 31, you will not be able to add and/or delete any dependent without a qualifying event.

Estimados Miembros :

Les queremos anunciar que la fecha de Inscripción Abierta es durante el mes de Febrero. Como ya ustedes saben la Inscripción Abierta es la fecha designada por el seguro médico en la cual se le dá a ustedes la oportunidad de incluir o eliminar sus dependientes que son elegibles. Los dependientes elegibles son : esposa/esposo, hijos menores de 19 años, e hijos entre las edades de 19-23 que sean estudiantes universitarios de tiempo completo. Para poder incluir sus dependientes elegibles nosotros necesitamos una copia del certificado de matrimonio, certificado de nacimiento (s), y una carta de la universidad para hijos mayores de 19 años. Para poder eliminar sus dependientes ustedes deben hacer la petición por escrito. Si ustedes no proveen a el Fondo de Salud con la información requerida antés del 31 de Marzo, ustedes no podran incluir o eliminar ninguno de sus dependientes al seguro sin un cambio elegible.

LOCAL 888 PENSION FUND

It’s never too late to think about the future. The UFCW Local 888 has a pension plan for both full time and part time members to help provide our members with an income after retirement. (The terms and benefits eligibility depend on each individual contract)

In order to expedite applications for retirement benefits, it is recommended that you contact the Pension Fund office at least four (4) months prior to your retirement date.

The Fund Office also needs a copy of the following documents:

1) Birth certificate; If you don’t have a copy of your birth certificate, the Fund will accept one of the following documents:
a) U. S. Passport;
b) Certificate of Citizenship;
c) Insurance policy over a period five (5) years;
d) Certified school records;
e) Hospital birth records;
f) Certificate of baptism as an infant;
g) Certificate of U.S. Armed Service;
h) Social Security records; or
i) Certified church records.
2) Beneficiary birth certificate;
3) Marriage certificate or divorce papers; and
4) Spouse death certificate (if deceased).

Nunca es muy tarde para pensar en el futuro. La Local 888 UFCW tiene un plan de retiro para los miembros que trabajan tiempo completo o parcial para ayudarlos ha tener un ingreso después de su retiro. (Los términos y la elegibilidad dependen de cada contracto)

Mensaje de la Gerente del Fondo

Estimados Miembros:

Para acelerar el proceso de las aplicaciones de beneficios de Pensión, es recomendable que usted se comunique con la oficina del Fondo de la Pensión al menos cuatro (4) meses antes del día de su retiro.

La Oficina del Fondo también necesita una copia de los siguientes documentos:

1) Alta de nacimiento; Si usted no tiene una copia de su acta de nacimiento, el Fondo aceptara uno de los siguientes documentos:
a) Pasaporte de los Estados Unidos;
b) Certificado de Ciudadanía;
c) Póliza de seguro con más de cinco (5) años;
d) Archivos certificados de la escuela;
e) Archivos del hospital de su nacimiento;
f) Certificado de Bautismo durante la infancia;
g) Certificado de los servicios de las Fuerzas Armadas de los Estados Unidos;
h) Archivos de la oficina del Seguro Social; y
i) Archivos certificados de la Iglesia.
2) Acta de nacimiento de su beneficiario;
3) Certificado de matrimonio o papeles de divorcio; y
4) Certificado de defunción de su esposo (a) (si ha fallecido).
